How Assisted Transition Helps Seniors

• We meet with the Senior and their family in person
• We assess their medical needs - including checking with the Family Doctor (if available)
• We help the family determine their financial position
• We take away the confusion and assist with finding the best option: Assisted Living, Alzheimer’s Communities, Independent Living, Skilled Nursing Homes, Home Care
• We locate facilities with openings and identify services within the Geographical Area where the Senior would prefer to live, and that are convenient for the family
• When appropriate, we form a Team of Associates that can help Seniors With Their Unique Needs for VA Aid & Attendance, Medicare/Medical, Life Insurance Policy Conversions
• We personally accompany the Senior and/or their Family on tours of the Facilities that best meet their needs

• We have personally toured, screened and evaluated every facility we represent
• We work with each Facility to insure that every need for our Seniors can be achieved before they contract with their chosen facility
• We follow up with the Senior and their Family to make sure they are happy with their choice
